Responsibilities
Acts as Service Coordinator on behalf of individuals with developmental disabilities. Develops and maintains linkages with community organizations and other supportive resources to enable assigned individuals to live and participate in the community, according to their personal goals, preferences and needs. Makes arrangements for the use of such services and supports as needed.

Qualifications
Must have an Associate's Degree in a health or human services field from an accredited college or university or a degree in nursing as a Registered Nurse. Must have at least one year of experience working with people with developmental disabilities or one year of experience as a service coordinator/case manager with any population. An applicant who has an Associate's degree or Bachelor's degree, or a minimum of 60 credits toward a Bachelor's degree in a field other than health or human services, may meet this educational requirement if a minimum of 20 college credits are in a health or human services field(transcript must be verified). Must have a valid New York State Driver's License, a reliable vehicle; meet VEHICLE OPERATOR REQUIREMENTS and agency standards to transport service recipients.
